Thyroid nodule, increased TSH, low wbc
I am a 23 year old Female with no significant past medical history. In October of 2016 I had a miscarriage and the OB found that my TSH was 13.4 and my Free Thyroxine is 0.84. He also palpated and felt an enlarged thyroid on the right side. He followed this up with an ultrasound that said that the thyroid was enlarged on the right side with increased vascularization, increased lymph nodes, and diffusely heterogenous. There were no nodules present. They put me on 100mcg of levothyroxine and scheduled a check up 6 months later. I went in to my primary Dr. recently where she ran tests and did another ultrasound. my WBC was 3.6, TSH 10.2, free thyroxine 0.78, and neutrophils 1.68. She then increased my snythroid to 125mcg. The ultrasound came back with this report:
Indication:NONTOXIC SINGLE THYROID NODULE

Findings:
Right lobe of the thyroid measures 6.8 x 1.9 x 2.2 cm. 5 mm nodules present.
Thyroid is diffusely heterogenous.
No abnormal cystic or solid nodules are seen in the isthmus.
Left lobe of the thyroid measures 5.6 x 2.1 x 2.3 cm. Thyroid is diffusely
heterogenous. No focal nodules are seen.
Impression:
1. Thyroid is enlarged and heterogenous. This nonspecific but could be secondary
to thyroiditis
2. 5 mm right thyroid nodule may well be benign but ultrasound follow-up is
recommended
I have an endocrinology appointment on June 30th. I guess my question in what is going on with me and what other tests should be run? There have been a lot of frustrations throughout this process of Dr.’s quitting and the apt not being rescheduled and other hiccups along the way that have made me feel nothing is moving at a very fast pace and no one is really explaining anything to me.

My opinion This is Hashimoto’s disease, this is a Cyst not a nodule , it seems benign (harmful) , in order to have a successful pregnancy your doctor will need to keep TSH under 3 which means you will need a higher dose if you get pregnant,
By the way it is a cyst not a nodule because nodules cause hyperthyroidism, which is not the case here

Your problem is hypothyroidism which means your thyroid gland is not producing enough thyroxine hormone, which is of no cause mostly or genetic predisposition ,
You are supposed to keep tracking your hormone levels, every 6 months and taking your medication regularly ,and monitoring your thyroid size with us every 3-4 months.
Avoid weight gain and practice any mode of exercise regularly .
Let me tell you that you as moving smoothly and fine with the improvement of your condition,
Hypothyroidism is a common to happen with females, and most probably its the cause of the miscarriage
After reaching the right level of T4 and TSH by adjusting the dose of the thyroxine , hopefully everything is going back to normal
Tests need to your Condition are Full blood count, T4, TSH, ultrasound, others tests to check specific circulating antibodies…

Continue the medication as advised along with regular followup to check for the level of thyroid hormones and first let it come to normal..
Continuously get monitored for nodule size..
If at all needed get a biopsy done..
