I have a 35 years old lady who is a patient of mine, she has been having pitting oedema in and off for 13 which is not painful but after a while resolves itself..cardiac echo, urinalysis, liver and kidney function tests, full blood count, abd uss are normal with the exception that her Hb is 9.8g/dl. No family or previous hx of bp or diabetes. We cant figure out what’s wrong with her..any help would be appreciated.

On and off pitting oedema have 2 main reasons
Allergic reaction or obstruction in blood blow related to specific body movement or posture which for example can lead to finding uterus cyst that pressure an artery in sitting only
Olso is the oedema systemic or just localised and where ?
Get her checked for venous stasis ..varicose veins by duplex and potassium sodium if not dine yet…
Plenty of water ..elevation and lymph drainage massage can help..

it can happen due to hypoproteinemia or anemia.
If starting on hematinics does not resolve the issue,then get her proteins checked.
Especially albumin,globulin and A:G ratio needs to be assesed.

THIS CAN BE DUE TO HORMONAL VARIATIONS,ESPECIALLY DISTURBED OESTROGEN LEVELS. OTHER POSSIBILITY IS OF A BLOCKAGE IN LOWER LIMB VEINS

lymphoedema or deep venous thrombosis can be the cause. get doppler studies done. application of magnesium sulphate and glycerine with leg elevation can help.
