I’ve had a lump in my forearm for about a decade that was diagnosed as an aneurysm of the left basilic vein. It gets larger when I play sports or lift weights. The doctor can perform an aneurysmectomy or leave it alone. What do you recommend?

If its been there for a decade and has not significantly increased in size,then you should leave it alone.
Only if it becomes painful or dramatically increases in size then you can think of aneurysmectomy. Wear compression bandages when you do heavy work or when you do sports.

The aneurysm is of significant size and needs surgical removal. If neglected..it can increase in size or may cause troubles later on. Better to get it removed.
Get a venous doppler repeated before going for surgery

THIS DOES SEEM TO BE A BIG ENOUGH LESION THAT CANNOT BE NEGLECTED. INSTEAD OF SURGICAL REMOVAL..LASER CAUTERISATION CAN BE HELPFUL. IT WILL PREVENT OPERATIVE AND BLEEDING RISKS
