these findings are usually seen in acute glomerulonephritis meaning infection of kidneys.
consult a nephrologist and start proper medications. he will be fine in few days, dont worry.
give him plenty of water and high protein diet.

GET HIS KIDNEY FUNCTION TESTS AND USG ABDOMEN DONE
